Deceased Information

  • Branch – If more than one location, select which branch is managing the service.
  • Salutation, Given Name, * Middle Name – include all middle names, Last Name, Suffix – if applicable, and Nick Name – This name will appear with surrounding quotations after the middle name(s).
  • Gender – The template color will change to either green or pink.
  • Marital Status
  • Date of Death – Use the calendar icon for quicker entry.

Family Interactive Edition

  • The system automatically generates a Family Username and Family password. (Both can be changed to what the funeral home or family prefers.) This allows the family to login to their loved one’s Book of Memories to view private condolences and flower cards, change the theme, approve any photos or stories (if the approval process is turned on), upload photos, share stories, and send e-thank-you cards to anyone who signs the guest book.
  • Family Interactive and Condolence Email List: Add the email addresses of any family members who would like to receive the family login information (and future login changes) and all condolences. If there is more than one address, click ‘Add E-mail.’ The condolences are sent once a day to email addresses entered. (All condolences are filtered for any inappropriate words.)

Place of Death

  • Copy Address From – Use the dropdown to pull the same address information from another source.
  • Place of Death – If the location is not contained within the dropdown, ‘Add.’ A pop-up window will appear. To learn how to add a location, please refer to Databases.
  • Country, State/Province, City – If the city location is not contained within the dropdown, Add . A pop-up window will appear. To learn how to add a city, please refer to Databases.

  • Hospice Facility Name – If applicable

Informant Information

  • Copy Address From – Use the dropdown to pull the same address information from another source.
  • Salutation, First Name, Middle Name – include all middle names, Last Name, Maiden Name – if applicable, and Suffix – if applicable.

  • Phone Number, Cell Phone Number

Next of Kin

  • Copy Address From – Use the dropdown to pull the same address information from another source.
  • Same as Informant – Check the box if the next of kin is the same as informant. If so, all informant information will automatically populate the fields below.
  • Salutation, First Name, Middle Name – include all middle names, Last Name, Suffix – if applicable, and Maiden Name – if applicable.
  • Email Address
  • Relationship to deceased
  • Phone Number and Cell Phone Number

Executor
