- Relation Type
- Relation to deceased
Choose all that apply:
- Predeceased
- Contact
- Next of Kin
- Informant
- Executor
Enter information for this family member:
- Salutation, First Name, Middle Name, Last Name, Suffix, and Maiden Name – if applicable.
- Country, State/Province, and City – if you do not see a city contained in the drop down list, ‘Add.’ A pop-up window will appear. To learn how to add a city, please refer to Databases.
- Address 1 & 2, Unit/Apt., and Zip/Postal Code
- Phone Number and Cell Number
- Email Address
- Birth Country, Province, City – if you do not see a city contained in the drop down list, ‘Add.’ A pop-up window will appear. To learn how to add a city, please refer to Databases.
- Employer
- Date of Birth
- Add Relation Tasks, Occupation and Age
- Save
