Fill in all applicable information:

  • Relation Type
  • Relation to deceased

Choose all that apply:

  • Predeceased
  • Contact
  • Next of Kin
  • Informant
  • Executor

Enter information for this family member:

  • Salutation, First Name, Middle Name, Last Name, Suffix, and Maiden Name – if applicable.
  • Country, State/Province, and City – if you do not see a city contained in the drop down list, ‘Add.’ A pop-up window will appear. To learn how to add a city, please refer to Databases.
  • Address 1 & 2, Unit/Apt., and Zip/Postal Code
  • Phone Number and Cell Number
  • Email Address
  • Birth Country, Province, City – if you do not see a city contained in the drop down list, ‘Add.’ A pop-up window will appear. To learn how to add a city, please refer to Databases.
  • Employer
  • Date of Birth
  • Add Relation Tasks, Occupation and Age
  • Save
