Vital Information

Most of the information in this section will be populated if First Call information was entered. The following are fields not included within the First Call.

• Maiden Name – If applicable.
• Race
• Of Hispanic Origin
• Video Link – Enter the URL of a third party company used for video tributes. (There is no option to upload a video directly into this system.)
• Social Insurance Number/Social Security Number
• Religious Denomination

Residence Information

• Copy Address From – Use the dropdown to pull the same address information from another source.
• Country, State/Province, and City – if the city is not contained in the dropdown list, ‘Add.’ A pop-up window will appear. To learn how to add a city, please refer to the Databases.
• Address, Apt/Unit, Zip/Postal Code
• County
• Length of Residence – In years

Birth & Death Information

• Place of Birth (Facility)
• Country of Birth, State/Province of Birth, City of Birth – if the city is not contained in the dropdown list, ‘Add.’ A pop-up window will appear. To learn how to add a city, please refer to the Databases.
• Date of Birth, Date of Death, and Time of Death
• Age at Death Years – The age will automatically populate once the date of birth and date of death have been entered.
• Age Less Than a year
• Hospital Code
• Birth Certificate Number and Birth Certificate Issuer
• Death Occurred at Hospital – If applicable, select hospital department

Next of Kin

These fields will be populated if information was entered in the First Call fields.
